How is 'the cloud' different from 'hosted'?

It seems to me that we've been using hosted computer systems for a long time. Some hosted solutions use server clusters (in fact, I'd think any good hosting company would provide this). So since we know hosted systems work, what's the worry about 'the cloud'?

Is there a functional difference or are we just rebranding something that's been around since the beginning of networks? (damn, I'm getting old and cranky... now you kids, get off my lawn!)
